Apley’s Distraction Test

The Apley’s Distraction test is used to detect the presence of ligamentous instability of the knee.

With the patient in the prone position, flex the patient’s knee to 90˚ while stabilizing the distal thigh against the table. Grasp the ankle with both hands and apply upward traction while rotating the tibia internally and externally.

A positive finding is excessive joint separation or excessive internal or external rotation, with or without pain, implicating involvement of joint restraints such as the collateral and cruciate ligaments.
